John Andrew Burlata, 54, of Chicago, passed on July 3rd 2021. He was the loving brother of Charles Michael Ballard(deceased) and a fond Uncle to Katherine Jane Ballard. John is survived by cherished family and friends. He is preceeded in death by his parents Jan & Ludmila Burlata, formerly from the Ukraine as well as long time residents of Chicago.John was a sweet and loving human. He loved bowling, church, sports, movies, nature and going shopping.
